In Traditional Chinese Medicine, the concept of a "pattern" is foundational, representing a comprehensive diagnosis that encapsulates the underlying cause of symptoms like carbuncles. A pattern goes beyond the superficial signs to the root imbalances in the body's Qi, Blood, and Organ systems.

Understanding the specific pattern—be it Toxic-Heat, Blood Deficiency, or another—is crucial because it informs a tailored treatment strategy. This approach ensures that therapy addresses both the manifestations and the internal disharmonies, paving the way for genuine, lasting healing.

Rapid (Shu), Full (Shi), Overflowing (Hong)

Tongue

The tongue is characteristically red or deep red, reflecting intense interior Heat. Prickly raised papillae (thorns) may appear, especially on the tip and centre, indicating Heat has become concentrated into toxin. The coating is yellow and dry, sometimes thick, showing Heat consuming body fluids. In severe cases progressing toward the Blood level, the tongue may become crimson (jiang), but in the typical Toxic-Heat presentation at the Qi level, bright red with dry yellow coating is the hallmark finding.

Tongue

The tongue is typically deep red or crimson, reflecting intense Heat in the Blood level. Prickly papillae (thorny projections) may be visible across the surface, indicating Heat toxin at its peak. The coating is thick and yellow, often dry due to Heat consuming body fluids. In severe cases, the tongue may appear swollen. Red spots, especially at the tip, are common, reflecting Heart Fire being stirred by the toxin. If the condition has been prolonged, the coating may become greyish-yellow or even brownish-black from dried, scorched fluids.

Diagnostic signs

Pulse

Fine (Xi), Floating (Fu), Wiry (Xian)

Tongue

The tongue body is typically pale and may appear slightly thin or small, reflecting the underlying Blood Deficiency. The coating is thin and white, which is consistent with an external pathogenic factor (Wind) that has not yet generated Heat. The tongue may appear slightly dry, especially at the edges, due to Blood's failure to moisten. If the condition is longstanding, slight peeling may begin to develop in patches, but a fully geographic tongue would suggest progression toward Yin Deficiency.
