Stomach churning is a common gastrointestinal symptom characterized by a sensation of agitation or movement within the stomach. It often presents as a rumbling or rolling feeling, sometimes accompanied by discomfort.
This sensation can be associated with various digestive issues, ranging from mild indigestion to more serious conditions. It is a manifestation of the body's response to different stimuli affecting the digestive system.
In Traditional Chinese Medicine (TCM), stomach churning is perceived through a unique perspective that focuses on the flow of Qi (vital energy) and the balance of internal organs. TCM practitioners consider this symptom as an indicator of disharmony or imbalance within the body's energy system.
Emotional stress, dietary habits, and the individual's overall health are key factors in understanding the root cause of stomach churning. TCM aims to identify these underlying patterns and restore balance, thereby alleviating the symptom.
TCM Patterns for Stomach Churning
Each pattern represents a distinct underlying imbalance that can cause stomach churning
Liver Qi Stagnation
Diagnostic signs
Wiry (Xian)
The tongue body is typically normal or light red. In early or uncomplicated cases, the tongue may appear entirely normal. The most distinctive feature is redness or a slightly purplish hue along the sides of the tongue, which correspond to the Liver zone in tongue diagnosis. Some patients show frothy saliva along the tongue edges. The coating is usually thin and white. If the stagnation has begun to generate Heat (a common progression), the sides may become redder and the coating may start to thin or turn slightly yellow. If there is concurrent Spleen involvement, the coating may become slightly greasy.

Rebellious Liver Qi
Diagnostic signs
Wiry (Xian)
In mild or early cases the tongue body may appear entirely normal with no significant changes. As the pattern develops, the sides of the tongue (corresponding to the Liver area) may become slightly red, reflecting the excessive upward and outward movement of Liver Qi generating mild Heat. The coating typically remains thin and white, as this is primarily a Qi-movement disorder rather than a Heat or Dampness pattern. If the pattern begins to transform into Liver Fire or invade the Stomach with Heat, the coating may develop a slight yellow tinge.
