Traditional Chinese Medicine (TCM) interprets purulent otitis not merely as a localized infection but as a manifestation of an imbalance within the body’s energetic system. In TCM, the ear is intimately connected with the Kidney, and thus, symptoms in the ear often reflect deeper systemic issues.
Patterns of disharmony, particularly those affecting the Liver and Gallbladder channels, are scrutinized to tailor an effective treatment strategy. Recognizing the underlying pattern, such as Liver Fire Blazing or Gallbladder Damp-Heat, is essential for successful intervention.

According to TCM, purulent otitis can arise from several patterns of disharmony. Predominantly, it is associated with the Excessive Heat, often from the Liver and Gallbladder Channel, which may manifest as Damp-Heat in the Liver or Stagnant Liver Qi transforming into Fire. These conditions create a fertile ground for the development of infections leading to symptoms like ear pain, discharge, and hearing issues. The tongue body is distinctly red, often most intensely along the sides (which correspond to the Liver and Gallbladder in tongue geography). In cases with stronger heat, prickly points or thorns may appear on the sides. The coating is yellow and dry when pure heat predominates. When Dampness is also present, the yellow coating becomes greasy or sticky rather than dry. In the Liver Fire sub-type, the coating may be thinner and drier, while in the Damp-Heat sub-type, it tends to be thick, yellow, and greasy.
