Commentary

Kouheliao LI-19 is not a major point. Its main function is to expel Wind so as to open the nose passage. 

Classical Sources

Zhenjiu Jiayi Jing (针灸甲乙经): First documented this point. The text describes its indications for corpse-like fainting (尸厥), inability to open the mouth, nasal sores, nasal polyps, nasal obstruction with loss of smell, and persistent nosebleeds.

Zhenjiu Zisheng Jing (针灸资生经): Records the combination of Kouheliao with Hegu, Lieque, and Jiache for treating lockjaw (口噤不开), and with Cuanzhu for corpse-like syncope (尸厥).

Zhenjiu Dacheng (针灸大成): Lists the comprehensive formula for nasal obstruction: Kouheliao, Yingxiang, Shangxing, Wuchu, Shuigou, Fengfu, Bailao, and Taiyuan.

How to Locate

Divide the distance between the nostril and the upper lip into three section. Renzhong DU-26 is located on the anterior midline, at the junction of the upper and the middle third. Kouheliao LI-19 is at the same level as DU-26, 0.5 cun lateral to it.

Caution

This point is located in a sensitive facial area near the facial artery and vein branches, as well as the facial nerve and infraorbital nerve plexus. Use shallow oblique or transverse insertion only. Avoid perpendicular deep needling. The area has many nerve endings, so needle gently to minimize patient discomfort. Due to the thin tissue and proximity to facial vessels, minor bruising may occur.

Expected Deqi Sensation

Local distension and soreness around the upper lip and nostril area. Sensation may radiate toward the nose or along the nasolabial groove. Due to the thin tissue and nerve-rich area, the sensation can be relatively sharp initially.
